All applications must be accompanied by the following documentation:

- Please print or type answers on the application
- All requested documents on the checklist must be attached to the application
- No faxed or emailed applications will be accepted
- This application should be postmarked by no later than 12:00 noon April
16, 2010

APPLICATION QUALIFICATIONS
Applicants will be judged on the following criteria:
- Compelling Essay
- Academic Achievement
- Evidence of Leadership
- Community Service
- Grade report/transcript (most recent high school transcript for seniors, college transcript for post-secondary students)
- Two letters of recommendation (non-family)
- Proof of current enrollment or acceptance in a college/university (course schedule/admission letter, if applicable)
- Must provide a minimum 500 word essay including:

- Please tell us about yourself. What is your definition of leadership? Describe your personal achievements and accomplishments. Explain any obstacles you have overcome in your life and how they have made an impact.
- Why do you wish to attend college? What field of study do you intend to pursue? How do you plan to distinguish yourself as a leader in the future?

PLEASE NOTE:
Applications missing any of these materials will not be considered. Letters of recommendation that arrive separately will not be accepted. Scholarship winners will be required to submit proof of full-time college enrollment and Financial Aid award information before awards are disbursed.
